2.  Please click on “Print Patient Forms” to open patient forms.pdf in Adobe Reader.  Press the print button to print all 9 pages.  Please complete paperwork and bring to your appointment:

Patient forms include:

1. Patient History
2. Financial policy form
3. Privacy policy consent form (required by law).  In order to treat you, we need to have the signed form on file before treatment commences.

3.  Please bring any of the following that relate to your reason for seeing the doctor:

• Lab results
• X-ray (reports only)
• MRI, ultrasound or Cat Scan reports

4.  Please bring your supplements, vitamins and herbs you are currently taking so the doctor can look at the actual labels.  There are hundreds of products now available and it is impossible to be familiar with all of them.

5.  If you have insurance, please either fax your card copy (front and back) to the office at 503-864-4941 before your appointment or bring it with you to your appointment.
